چکیده
migration in the treatment of anastomotic leak Toshniwal J et al. report about the use of the OTSC System to anchor a fully covered self-expandable metal stent to prevent stent migration. The patient underwent distal esophagectomy with gastric pull-up. The stent was placed to a postoperative anastomotic leak in the esophagus. However, the stent partially migrated into the stomach. The stent was then repositioned onto the leak. The OTSC System was placed using the OTSC Twin Grasper to grasp the stent edge and suction. After application the OTSC clip fi xed the stent to the esophageal wall. Follow-up showed successful closure of the anastomotic leak. The authors conclude that the placement of the clip was easy, fast and prevented stent migration effectively. The fi xation of stents is not a common indication for the OTSC System and there is only very limited experience. Combination of the "bear claw" (over-the-scope-clip system) and fully covered stent for the treatment of post-operative anastomotic leak.
The “bear claw” or over-the-scope clip (OTSC) system (Ovesco Endoscopy, Tübingen, Germany) is an innovative clipping device made of superelastic biocompatible nitinol [1–3]. This clipping device was developed to close wall defects of the luminal gastrointestinal tract, such as perforations, anastomotic leaks, fistulas, and large defects occurring after endoscopic full-thickness resection [1]. متن کاملRadiologic imaging of over-the-scope-clips can be misunderstood: the "pooling sign".
The conventional treatment of gastrointestinal defects, including perforations, fistulas, and anastomotic leaks, is surgery, which is associated with significant morbidity and mortality [1–6]. The over-thescope clip (OTSC) (Ovesco Endoscopy AG, Tübingen, Germany) conforms to the shape of a cup after deployment (●" Fig.1 and●" Fig.2) and provides more durable closure than standard clips because ...
متن کاملMultipurpose use of the over-the-scope-clip system ("Bear claw") in the gastrointestinal tract: Swiss experience in a tertiary center.
AIM To evaluate the outcome of over-the-scope-clip system (OTSC) for endoscopic treatment of various indications in daily clinical practice in Switzerland. METHODS This prospective, consecutive case series was conducted at a tertiary care hospital from September 2010 to January 2014. Indications for OTSC application were fistulae, anastomotic leakage, perforation, unroofed submucosal lesion f...
متن کاملImplantation of a colorectal stent as a therapeutic approach in the treatment of esophageal leakage
BACKGROUND While the mortality of esophageal surgery has decreased due to technological advancements, there is still a complication rate of about 30%. One of the main complications is the anastomotic leakage associated with a significant rate of morbidity and mortality. To close the leakage the efficacy of self-expanding stents (SES) has been shown in different studies. However, the high rate o...
متن کاملFull covered self-expandable metal stents for the treatment of anastomotic leak using a silk thread
To evaluate the safety and effectiveness of fixation of the fully covered self-expandable metal stent (SEMS) placement using a silk thread for complete closure of an anastomotic leak. An anastomotic leak is a life-threatening complication after gastrectomy. Although the traditional treatment of choice was surgical re-intervention, an endoscopic SEMS can be used alternatively.During the study pe...
